When a student desires to change or apply to another major or degree within the University, the student must complete a Change of College, Major or Degree Option Form. The Change of College, Major or Degree Option Form may be found online or at OU Health Sciences Recruitment and Admissions.
As instructed on the form, the student must complete the necessary sections, secure required information and signatures from the accepting college/program and the releasing college/program, and submit the form to OU Health Sciences Recruitment and Admissions.
